What to bring on your appointment

Paperwork

Please bring your completed paperwork to your appointment. We ask that you arrive 20 minutes prior to your scheduled appointment. This will allow for processing of new patient information. If you have not completed your paperwork please arrive 30 minutes before your appointment to complete it.

Imaging

You must bring all relevant imaging (MRIs, CTs, and X-rays, etc.) to your appointment, including the radiologist’s interpretation.  This is essential for our review and to allow for timely medical care.  Your referring physician, or the imaging facility, may have attempted to send the images but, in our experience, this is often difficult to accomplish.  We reserve the right to re-schedule your appointment if we don’t have your imaging studies.

Insurance Card and Payment Methods

You should bring your insurance card and driver’s license or another form of picture identification to your appointment. We are legally required to collect your co-payment at the time of service. Our office accepts cash, check, Visa, MasterCard, American Express and Discover. If you do not have your co-payment at the time of your visit, you will need to reschedule your appointment.
