What to bring on your appointment

Paperwork

Please bring your completed paperwork to your appointment. We ask that you arrive 20 minutes prior to your scheduled appointment. This will allow for processing of new patient information. If you have not completed your paperwork please arrive 30 minutes before your appointment to complete it.

Imaging

Please bring any MRIs, CTs and X-rays to your appointment, including the radiologist’s interpretation. Your referring physician may have attempted to send the X-rays but in our experience, this is often difficult to accomplish in a timely manner. Please make all efforts to have the imaging studies for our review to allow for timely medical care. You will be asked to re-schedule your appointment if we don’t have your imaging studies.

Insurance Card and Payment Methods

You should bring your insurance card and driver’s license or another form of picture identification to your appointment. We are legally required to collect your co-payment at the time of service. Our office accepts cash, check, Visa, MasterCard, American Express and Discover. If you do not have your co-payment at the time of your visit, you will need to reschedule your appointment.
