General Information

Please let us know if you desire a chaperone present during your visit and examination.

Please make all efforts to have a translator (trusted friend or family member) available at your visit or notify the office if you have other specific needs. International patients who we have arranged travel for will have necessary services provided.

We ask that you arrive 20 minutes prior to your scheduled appointment. This will allow for processing of new patient information.

If you need to reschedule or cancel this appointment, we would appreciate at least 24 hours notice. Failure to do so may result in a $25 fee.

Imaging Studies

Please bring ANY MRIs, CTs and X-rays, including the radiologist's interpretation.

Your referring physician may have attempted to send the X-rays but in our experience this is often difficult to accomplish in a timely manner. Please make all efforts to have the imaging studies for our review to allow for timely medical care. You will be asked to re-schedule your appointment, if we don’t have your imaging studies.

Insurance Card and Payment Methods

You should bring your insurance card and driver’s license or other form of picture identification to the appointment. Most insurance plans require co-payment at the time of service. Our office accepts cash, check, Visa, Mastercard, American Express and Discover. If you do not have your co-payment at the time of your visit, you may be asked to reschedule your appointment.

Initial Consultation

  • You are being seen by BNA neurosurgeons for a different condition than your previous appointment

2. Questionnaires
Patients who are being seen for any other condition not related to the spine conditions do not need to complete these forms.

You will need your BNA patient ID number to log in. If you were not given or forgot your ID number, please email us at research@bnasurg.com or call at 303-938-5700.

Follow-up Visit

Please complete these forms if you had surgery or any other treatment and it has been 3 or more months since your last visit. Please complete these forms any time you come for your follow-up visit or at 3 months, 6 months, 12 months, 2 years, 3 years, 4 years, 5 years after the surgery. You don’t need to complete this form at your immediate postoperative appointment, which usually occurs at 7 to 14 days after surgery.
